Outdrives get neglected because they can't be seen. Antifoul, even if it looks sound, is only active for a couple of years at best.
Ideally have your drives re-antifouled each time your hull is done.These old Volvo drives had been neglected. The picture shows them after they had been prepared for antifouling.
If they had been looked after more regularly the time taken in preparation would have been much less.

This boat was for sale. Not only is the vendor giving a better first impression, the new owner will be getting a pair of drives that are well protected.
There is also a clear coating available from International that can be applied over antifoul or on the manufacturers paint that is so slippery the foul stands no chance of adhering.
Worth considering for high foul areas.
